5/22/2023 EnchantaVaughn Approved (AM) Upon arrival, client was supported for being in his seat, completing his work, and avoiding distractions. Client was focused and on task. Client had one on one with counselor. Client was inqured about his morning and meeting objectives. Client said he was working and being a good student. Client said he was looking forward to today’s activities. Client confirmed he didn’t need anything from QMHP but to take a break when he finishes his work. Counselor agreed and prompted client to utilize coping strategies to deal with difficult tasks. Client agreed to try his best to avoid aggressive and impolite behaviors and comply to both classroom and school rules at all times. (PM) Client was observed during lunch. Client had appropriate interaction with peers. Client was having appropriate conversation. Client was respectful of staff and peers. Client was helpful to his peers, as evidenced by him picking up trash that wasn’t his. Client was courteous to authority requests and demands. The client was provided with positive feedback as he displayed a realistic understanding of his recurrent pattern of mental illness symptoms. Client was able to stay in class without any incidents. The client is beginning to show more trust in others as demonstrated by increased social interaction. The client has shown an interest in relating to others in a more appropriate manner. The client has become less defensive and has not shown any recent incidents of unreasonable anger. The client identified a variety of successes and difficulties in social settings and was provided with support and feedback in this area. Individual therapy was scheduled to assist client in adjusting to his improved performance. (EOD) Client was reported following directions and staying on task. Client was provided with feedback about how his behavior or verbal messages have an impact on others. Client expressed positive “I statements” in academic setting (e.g. I will participate in all assignments and I will wait my turn). Client was reminded of goals and objectives. Client was praised for his cooperation and for being on task. Client engaged in group discussion utilizing team building skills and active listening skills while interacting with his peers during specials. Client expressed positive thoughts. Client showed excellent teamwork by sharing and taking turns. Client displays appropriate social skills in group setting. Client utilized his breaks appropriately, hugging QMHP before returning back to class. Client was seen exhibiting good behaviors. Client was observed on track with his expectations. Client was supported for having a good day. Client was attentive and well-behaved during dismissal. Client met objectives, as monitored and reported.
